GENERAL SURGERY INFORMATION
Dear Client:
Thank you for scheduling your pet's surgical/dental procedure with us.
Attached are two copies of our hospitalization and surgical release form which provides an estimate, explains our hospital policy, requests your authorization to perform the scheduled procedure, and explains the preanesthetic protocol considered necessary. Please sign one copy and bring it with you on the scheduled date. The other copy you may keep for your reference.
Food and water restrictions are necessary prior to anesthesia. Food should not be offered after 7:00 P.M. the evening prior to and water should not be offered after 7:00 A.M. the morning of the anesthetic procedure.
Admission for surgical/dental patients is 8:00 A.M. Please allow approximately 15-20 minutes for the admission process and for a brief examination with one of our doctors.
